Synergy Home Care provide at home medical care, with Register Nurse, LPN ,personal care aides and certified nursing assistants, based on client’s needs from four to 24 hours a day, seven days a week.

Visits can be provided for Long Distance Families on an “as needed†basis to monitor the ongoing progress of the Client and care plan, and make adjustments as may be beneficial to the Client’s ever changing needs, either in the home or a health care facility.
